Raltegravir potásico

Riesgo muy bajo

Seguro. Compatible. Mínimo riesgo para la lactancia y el lactante.

Antirretroviral inhibidor de la integrasa. Indicado en combinación con otros dos fármacos antirretrovirales más en el tratamiento de la infección por el virus de la inmunodeficiencia humana (VIH 1) y en la profilaxis de la transmisión vertical (embarazo/lactancia) (Puthanakit 2018, Westling 2012) y postexposición accidental. Administración oral en 2 dosis diarias.

Se excreta en leche materna en cantidad clínicamente no significativa y los niveles en plasma de los lactantes fueron muy bajos o por debajo del límite de detección. (Aebi 2022, Feiterna 2020)

Es un fármaco bien tolerado y con poca frecuencia de reacciones adversas graves (MSD 2019, AEMPS2018, Clarke 2018, Ribera 2010) incluidos neonatos, lactantes y niños (Clarke 2018, Ripamonti 2016, Nachman 2015 y 2014).

Se utiliza en recién nacidos no prematuros y a partir de los 3 Kg de peso (Trahan 2019, MSD 2019, AEMPS 2018). La OMS contempla el uso neonatal (Clarke 2018, WHO 2016, WHO 2014). Se recomienda a partir de las 2 a 4 semanas de vida en las pautas de profilaxis postexposición no ocupacional a VIH (CDC 2016, WHO 2014)

En los recién nacidos prematuros tiene una semivida de eliminación más larga, de entre 15 y 22 horas (Kreutzwiser 2017)
