Riesgo muy bajo

Seguro. Compatible. Mínimo riesgo para la lactancia y el lactante.

El tenofovir disoproxil fumarato (TDF) y el fumarato de alafenamida de tenofovir son profármacos del Tenofovir. Es un inhibidor de la transcriptasa inversa análogo de nucleótido (ITIN) con actividad antiviral frente al virus de la inmunodeficiencia humana (HIV-1) y al virus de la Hepatitis B (VHB). Indicado en el tratamiento del SIDA y de la hepatitis B crónica y en la profilaxis pre-exposición VIH. Administración oral una vez al día durante meses o años. Autorizado en VIH a partir de los dos años y fuera de ficha técnica en menores de 2 años.

Se excreta en leche materna en cantidad clínicamente insignificante. (Kayes 2022, Erturk 2019, Mugwanya 2016, Palombi 2016, Benaboud 2011)

No se han observado problemas en lactantes cuyas madres lo toman (Kapito 2021, Marcellin 2016, Ganne 2013, Gouraud 2012, Pan 2012), salvo algún caso de diarrea leve. (Mugwanya 2016)

No hubo tampoco afectación de los marcadores de mineralización ósea (Floridia 2016 y 2016) ni de los índices de crecimiento a los 6 y 12 meses. (Liotta 2016)

Los niveles de tenofovir en plasma u orina de dichos lactantes fueron indetectables o muy bajos. (Kayes 2022, Erturk 2019, Waitt 2017 y 2018, Mugwanya 2016, Palombi 2016, Gouraud 2012)

Tras la administración en forma de gel intravaginal, las concentraciones en plasma y leche maternos y en plasma del lactante fueron indetectables o muy bajas y los efectos secundarios en los lactantes leves o inexistentes. (Noguchi 2016)

Diversas sociedades médicas y consensos de expertos consideran seguro el uso de esta medicación durante la lactancia. (Hu 2019, Wong 2018, Mugwanya 2017, Seidman 2017, Mofenson 2017, Terrault 2016, UNICEF 2016)
