氯噻酮

Limited compatibility

Unsafe. Moderate/severe adverse effects. Compatible under certain circumstances. Follow-up recommended. Use safer alternative or discontinue breastfeeding from 5 to 7 T ½ . Read Commentary.

It is a diuretic used to treat hypertension or edema caused by heart failure, kidney failure, liver cirrhosis, estrogen therapy, and other conditions. Oral administration.

It is excreted in breast milk in an amount that could be significant. (Mulley 1978)

In the past it was used to inhibit lactation. (Emran 1966)

Long-term treatment with diuretic drugs (particularly those Thiazides with long lasting effect and loop-acting drugs) may inhibit lactation. Use a lower dose as possible, especially during the first postnatal month.

Its long half-life and the amount that is excreted in milk make the use of an alternative advisable.

However, the American Academy of Pediatrics considers it a medication generally compatible with breastfeeding. (AAP 2001)

Alternatives

  • Chlorothiazide (Safe product and/or breastfeeding is the best option.)
  • Furosemide (Fairly safe. Mild or unlikely adverse effects. Compatible under certain circumstances. Follow-up recommended. Read Commentary.)
  • Hydrochlorothiazide (Safe product and/or breastfeeding is the best option.)

Suggestions made at e-lactancia are done by APILAM team of health professionals, and are based on updated scientific publications. It is not intended to replace the relationship you have with your doctor but to compound it. The pharmaceutical industry contraindicates breastfeeding, mistakenly and without scientific reasons, in most of the drug data sheets.
