アダリムマブ

Riesgo muy bajo

Seguro. Compatible. Mínimo riesgo para la lactancia y el lactante.

Anticuerpo monoclonal humano IgG1contra el factor de necrosis tumoral usado en enfermedades autoinmunes como la artritis reumatoide, artritis idiopática juvenil, espondilitis anquilosante, artritis psoriásica, psoriasis, hidradenitis supurativa, enfermedad de Crohn, colitis ulcerosa y uveitis.
Administración por vía subcutánea cada una a dos semanas.

Se excreta en leche materna en cantidad clínicamente no significativa (Matro 2018, Fritzsche 2012, Ben-Horin 2010) y no se han observado problemas en lactantes cuyas madres la tomaban (Matro 2018, Dall'ara 2016, Julsgaard 2013, Fritzsche 2012, Mishkin 2006, Mahadevan 2006, Vesga 2005). Los niveles en plasma de dichos lactantes fueron indetectables o muy bajos (Julsgaard 2013, Fritzsche 2012).

Por su naturaleza proteica es inactivado en el tracto gastrointestinal, no absorbiéndose, (biodisponibilidad oral prácticamente nula), lo que dificulta o impide el paso a plasma del lactante a partir de la leche materna ingerida (Lactmed, Rademaker 2018, Götestam 2016 , Witzel 2014, Butler 2014, Mervic 2014), salvo en prematuros y periodo neonatal inmediato, en los que puede haber mayor permeabilidad intestinal (Sammaritano 2020).

Posiblemente convenga evitar su uso en la primera semana tras el parto: puede haber paso significativo de IgG a leche hasta los primeros 3 - 4 días postparto. Se ha publicado un caso de neutropenia congénita precoz en un recién nacido cuya madre tomaba adalimumab por una enfermedad de Crohn e isoniazida por una tuberculosis (van den Broek 2018).

No se han detectado problemas en lactantes a cuyas madres se les administraban otros anticuerpos monoclonales similares, como belimumab, bevacizumab, infliximab, rituximab, tocilizumab o ustekinumab (Bar-Gil 2021, LaHue 2020, Saito 2020, 2019 y 2018, Klenske 2019, Mugheddu 2019, Krysko 2019, Matro 2018, Bragnes 2017, Hyrich 2014, Danve 2014).

El fabricante (Amgen 2017, EMA 2010) considera seguro el uso de esta medicación durante la lactancia.
Numerosos expertos y sociedades científicas de Reumatología, Gastroenterología y Dermatología consideran de muy bajo riesgo y, por tanto, compatible su uso durante la lactancia (Hale, Owczarek 2020, Sammaritano 2020 y 2014, Lamb 2019, Mahadevan 2019 y 2013, Picardo 2019, Amin 2018, Nguyen 2016, Flint 2016, McConnell 2016, Gotestam 2016, Huang 2016 y 2014, Briggs 2015, Grunewald 2015, Damas 2015, van der Woude 2015 y 2010, Nielsen 2014, Witzel 2014, Butler 2014, Schulze 2014, Yarur 2013, Bae 2012, Fischer 2010).
